Medical case management is a collaborate process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual’s health and vocational needs, using communication and available resources to promote quality, cost-effective outcomes.

VMS Medical Case Managers assist injured workers with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. The case manager works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying the patient’s health problem.

In addition, the case manager helps patients through conferences to understand, accept, and follow medical recommendations and provides services planned to restore patients to optimum social and health adjustment within the patient’s capacity. Furthermore, the case manager utilizes community resources to assist patients to resume life within their communities or to learn to live within the limits of their disability. Finally, the case manager prepares appropriate patient histories, service plans, and reports.
